Localities and Neighbourhoods is a vision of how to improve health and wellbeing, address health inequalities and prevent ill health on a locality and neighbourhood level. Integrated care, delivered in the community, will lead to people accessing care when they need it, receiving personalised care from integrated neighbourhood teams when required and where we enable people to stay well for longer.
A key component of this will be to support residents and communities to build resilience and lead their wellbeing. This will require a culture shift to an integrated, collaborative way of working, with professionals working together across organisational boundaries with the person.
Localities and Neighbourhoods - 2024/25 Priorities
To design, develop and implement the Tower Hamlets integrated neighbourhood team model
To support each neighbourhood to determine its own priorities
Build structures that support the resident voice to influence decision making and resource allocation at a neighbourhood level
Enable community organisations and health and care partners to work collaboratively to ensure accessibility for unheard groups
To develop and integrate social, welfare, legal advice as a core service in healthcare and other settings in Tower Hamlets to reduce health inequalities
