An integrated information and advice service across health and social care, as well as a directory of voluntary and community services - joined-up, consistent and accessible for all Tower Hamlets residents.
Tower Hamlets Connect was commissioned by THT and coproduced with residents and community organisations. It is delivered by a consortium of 9 local voluntary sector organisations with Age UK East London as the lead provider.
The service provides free, independent, quality-assured information, advice and advocacy across health, social care and social welfare to resolve issues early. Residents who require adult social care support will be triaged by the helpline team and referred onwards.
This service is a key driver in improving and supporting the health and wellbeing of Tower Hamlets residents; to remain independent and healthy for longer by providing early help, better coordination of service delivery, reducing duplication across services.
Tower Hamlets Connect is delivered by Age UK East London, the lead provider, and its consortium partners: Apasen, Bromley by Bow Centre, Deaf Plus, Island House, Limehouse Project, Positive East, Real DPO and Tower Hamlets Law Centre.
