We would like anyone who has used local health and social care services in the last 12 months to share their experience – good or bad – with us to help improve services to meet local people’s needs. The link to the survey is below:

The programme is ongoing so there is no deadline, and it can be completed as many times as needed. At the end of each quarter, we produce reports of our findings and share these reports with the service providers, commissioners, and the local authority.
