Celebrating personalised care in Tower Hamlets
Partners and staff across the Tower Hamlets primary care network, community teams, local charities and voluntary organisations with an interest in personalised care in the borough came together at an away day on Thursday 3 November to celebrate the implementation of the new NHS England Personalised Care model and to launch the Personalised Care Community of Practice Forum.
The new model for personalised care, takes a whole-population approaches to supporting people of all ages and their carers to manage their long-term physical, mental health and wellbeing and to live well with their condition. It enables people to make informed decisions with greater choice when their health changes, shifting from a ‘professionals know best’ approach towards a ‘patient knows best’ approach to managing their care.
Multiple Personalised Care Practitioner roles have been introduced across the borough to support and enable this paradigm shift and to support people to have more control over the care they receive.
The away day also launched the new Personalised Care Community of Practice Forum, a bi-monthly network in which speakers have a platform where they can inform and advise staff working with residents of Tower Hamlets on the help and care that is available to them.
The away day came to an end with an awards ceremony, marking the good work done by colleagues involved in creating personalised care plans for patients and service users taking into consideration the wider determinants of their health.

· Care Navigator award: went to the whole team for the humility they showed when asking to opt out of voting, as they all felt valued member of a collective team.
· Primary Care Network innovation award 2022: Tower Network PCN 9
